Abstract. Adenoid cystic carcinoma in the oral cavity is an uncommon salivary gland malignancy that has a propensity for perineural spread. A high-grade variant is evidenced by an abundance of pleomorphic cells, loss of the classic biphasic epithelial-myoepithelial growth pattern, and comedonecrosis, as well as elevated Ki-67.

Similar to the salivary gland counterparts, adenoid cystic carcinomas of the breast are characterized by t(6;9)(q22-23;p23-24) translocation resulting in a MYB-NFIB fusion gene. The t(12;15)(p13;q25) translocation resulting in ETV6-NTRK3 gene fusion (answer choice D) is seen in secretory carcinoma of the breast. The adenoid cystic carcinoma arising in the external auditory canal is a rare tumor, as other malignant neoplasias of the EAC and middle ear. The early diagnosis of these malignant tumors of the ear is very important to improve the prognosis. Adenoid cystic carcinoma generally presents in young or middle-aged adults with a mean age of 40 years, but can occur in the first decade of life. There is a slight predilection for women.
